Our Community Paramedic Division’s Mobile Integrated Healthcare (MIH) initiatives are designed to meet people where they are—literally and figuratively. By expanding the role of paramedics beyond emergency response, we provide proactive, compassionate care that helps individuals stay healthy, safe, and supported at home.

MIH bridges the gap between emergency services, primary care, and social support—reducing unnecessary hospital visits while improving quality of life across our community.

What Is Community Paramedicine?

Community Paramedicine is an innovative healthcare model that allows specially trained paramedics to deliver non-emergency care, follow-up services, and health education outside of traditional hospital settings.

Our community paramedics work closely with physicians, nurses, social workers, and local agencies to address both medical and non-medical needs.

How We Help

Our Community Paramedic Program focuses on prevention, access, and continuity of care. Services we are developing include:

  • Telehealth with Mobile Integrated Healthcare (MIH)
  • Chronic disease monitoring (such as diabetes, heart failure, or COPD)
  • Medication checks and education
  • Fall Risk Reduction, Assessment, and Treatment (FRRAT) Program
  • South Park Advanced Resource Care (SPARC) Program
  • Palliative and End-of-Life supportive care
  • Care coordination and referrals to primary care, mental health, or social services

  • Frequent 911 callers who would benefit from proactive care
